Insurance plan participation
Candlewood Center for Women's Health participates in most managed care plans and sees patients with government as well as traditional insurance. To learn if a particular physician or clinician participates with your plan, please contact your insurance company.
At your initial visit, you will be asked to present your insurance card, photo ID, and to complete new patient paperwork. After scheduling your first appointment you will be mailed the necessary new patient forms or you may download the necessary forms here. To save time during your first appointment, please complete the new patient paperwork ahead of time and bring it with you.
About insurance and payments
You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or our physicians do not participate with your health plan; full payment also is expected at the time of service.
For your convenience, we accept cash, check, MasterCard, VISA, Discover and American Express. All returned checks will be assessed a $25.00 service charge. If you are an existing patient with a balance due, you can also pay your bill online.
