Gynecology & Obstetrics asks that you complete your medical history in advance through our electronic medical records patient portal.

For your convenience, we also provide the following forms that you may download and complete before visiting us. If you choose not to utilize this method, please arrive to 30 minutes prior to your scheduled appointment time to complete the electronic medical records entry in our office. At regular intervals, you will be asked to update this information.
