Forms & Policies

To serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen.

At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and complete new patient paperwork. You may also download our new patient paper work and bring it with you at the time of your visit.

To Cancel or Reschedule

If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time or you may be subject to a cancellation fee.                                                                    

After Hours and Emergencies

Our Medical Team is available 24 hours a day, seven days a week including holidays to handle your emergencies. If you are in labor, or have an urgent illness, call the office number where you are usually seen and a Women’s Health Group practitioner will respond.

For prescription refills, laboratory results or other non-emergency needs, please call during regular office hours.

You may choose any one of our convenient office locations for your first visit. We recommend that you continue your visits at the same office.


For your convenience, Women’s Health Group provides the following forms that you may download and complete before visiting us. Otherwise these forms will need to be filled out upon arrival at our office so please come 15 minutes earlier to do that. At regular intervals, you will be asked to update this information.
