Forms & Policies

For your convenience, Sharon OB/GYN Associates provides the following forms that you may download and complete before visiting us. Otherwise the appropriate forms will need to be filled out upon arrival at our office. We ask that all patients arrive 15 minutes prior to their scheduled appointment time to complete this paperwork. At regular intervals, you will be asked to update this information.

The forms above are in PDF format and require Adobe Acrobat or Adobe Acrobat Reader. Acrobat Reader is downloadable for free on Adobe's website. Interactive forms opened and completed in Safari will not save changes made to the PDF.

These policies are provided for your reference:

• Prescription refill policy: All prescriptions are called in by the end of the day. Prescriptions are not filled on weekends or after office hours.
