Forms & Policies

For your convenience, S.H.E. Medical Associates provides the following forms that you may download and complete before visiting us. Otherwise, the applicable forms will need to be filled out upon arrival at our office. Should you opt to complete them at our office, please arrive at least 15 minutes prior to your appointment. At regular intervals, you will be asked to update this information.
