Forms and Policies

For your convenience, Shoreline OB GYN provides the following forms that you may download and complete before visiting us. Otherwise the applicable forms will need to be filled out upon arrival at our office so please arrive 15 minutes early to do this. At regular intervals, you will be asked to update this information.

The forms above are in PDF format and require Adobe Acrobat or Adobe Acrobat Reader. Acrobat Reader is downloadable for free on Adobe's website. Interactive forms opened and completed in Safari will not save changes made to the PDF.

Prescription Refill Policy:

Please allow 48 business hours for prescription refills.  After hours and weekend refills are for emergency prescriptions only.
