To Schedule an Appointment
To serve you in the best way possible, we work by appointments and can offer appointments within 24 hours of your call. Please let us know of any issues that may necessitate being seen sooner. Same day appointments are available for urgent problems.
At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and or deductibles that may be due, and to complete new patient paperwork. To save time during your first appointment, you may download [Forms & Policies] and complete our new patient paperwork ahead of time and bring it with you to your appointment.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time. There is a $40.00 fee applicable for non emergent late cancellations and no shows.
In Case of Emergency
After office hours all calls are answered by our office’s answering service. For non-emergent calls our voice mail system is available.
