If you are a new patient to Taylor Associates Obstetrics and Gynecology, please print all of the forms and documents below; read the Notice of Privacy; and complete all these forms and bring them with you to your appointment.

If you are an established patient and have updates to your medical history, insurance, residence, etc., please print out the applicable forms and note any changes. It is very important that you complete these forms and bring them to your next scheduled visit.
