To Schedule an Appointment

To serve you in the best way possible, we work by appointments. For patients with emergent or urgent issues same-day visits are available. Please let us know of any issues that may necessitate how soon you need to be seen.

In order to provide you with the best possible care, we need the most up-to-date information about you. If you are a new patient or an established patient who has not visited our office within the past three months, we will need a registration form updated at your next appointment.

You will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. To save time during your first appointment, you may download Forms & Policies and complete our new patient paperwork ahead of time and bring it with you. Otherwise, please arrive 15 minutes before your scheduled appointment to fill out the necessary forms.

We do our best to keep to appointed schedules, but medical emergencies and unexpected events do occur. If that happens and it delays your appointment, we promise to keep you informed and let you know of other options to fit your needs. Always feel free to call ahead to check for any known delays before coming in for your appointment.

To Cancel or Reschedule

If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment within 24 hours of your scheduled appointment time.

In Case of Emergency

After hours, please call our main office number (203) 374-0404 you will be prompted and routed to our answer service who will relay your message to the on-call physician.
