To Schedule an Appointment

To serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen. To schedule an appointment of a non-urgent matter online, you can do so here or call our appointment line at: 203-644-1150.

At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork to bring with you.

To Cancel or Reschedule

When an appointment time and date have been personally scheduled for you, we have allocated the Physician’s/Clinician’s time to serve you. We understand that unexpected events occur and you may need to change your appointment; however, we do request prior notice of at least 48 hours. A $40 service charge will be assessed if we have not received the appropriate notice.
