To Schedule an Appointment
To serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen.
At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork ahead of time and bring with you to your appointment. Otherwise we ask new patients to arrive 10 minutes early to complete all necessary paperwork. Having patients ready to be seen at their designated times is necessary to maintain an efficient schedule. Your assistance in this will benefit everyone.
If you arrive more than 10 minutes late, there is a chance that you cannot be seen. We will make every effort to fit you back into the schedule, however other patients who were on time may need to be seen before you, or we may have to ask you to reschedule.
To Cancel or Reschedule
The time for your scheduled appointment is set aside for you. If you miss your appointment, we try to understand that circumstances are not always in your control. A courtesy call to cancel or reschedule 24 hours in advance is appreciated and will help you avoid the potential for a $40.00 “No-Show” fee. Accessing his fee not reimbursed by your insurance company is our policy for patients who have missed 2 or more appointments without adequate notification.
If you have, or anticipate, having your period at the time of a pelvic exam, please call the office to determine if you will need to re-schedule.
