Schedule an Appointment
To serve you in the best way possible, our providers see patients by appointments. Please notify us of any issues that may necessitate how soon you need to be seen; patients can expect a same day appointment for urgent problems.
At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork ahead of time and bring it with you to your appointment.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time.
In Case of Emergency
We can be reached through our answering service outside of regular office hours. Please call our main number (203) 944-0242 and dial “0” at anytime for the operator to contact the CNM on call.
Insurance plan participation
Circle for Life Midwifery participates in many managed care plans and sees patients with government as well as traditional insurance. Some insurance plans may require that you obtain a referral from your regular doctor (PCP) before you visit us if we are not your designated gynecologist. To learn if a particular physician participates with your plan, please contact your insurance company.

Billing and Payments
You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or our physicians do not participate with your health plan, full payment also is expected at the time of service.
For your convenience, we accept cash, check, Mastercard, VISA, and Discover. All returned checks will be assessed a $25.00 service charge. If you are an existing patient with a balance due, you can also pay your bill online.
