To Schedule an Appointment
We suggest that you call 2-3 months prior to the time you wish to be seen for a routine exam. Please let us know of issues that may indicate that you need to be seen sooner. If you are having a problem, we will be happy to accommodate you. You may also request an appointment online.
At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. Your visit will be much more timely and efficient if your paperwork is completed before you arrive. These forms may be downloaded, [Forms & Policies] mailed or faxed to you in advance. If you anticipate completing the forms while in the office, please allow 30 minutes prior to your appointment to do so.
To Cancel or Reschedule
If you cannot keep your appointment, please give us at least 24 hours notice. Additionally, if you will be unavoidably late for your appointment, please call us to let us know. If you arrive very late, we may need to reschedule your appointment. There will be a $45 charge for "no-shows" if you miss your appointment.
Our telephone hours are from 8:30 AM to 1:00 PM and 2:00 PM to 4:30 PM. In an emergency and between 1:00 and 2:00pm, we can be reached through the answering service.
If you are a new patient to Progressive Women's Health, please print all of the forms and documents below; read the Notice of Privacy; and complete all these forms and bring them with you to your appointment.
If you are an established patient and have updates to your medical history, insurance, residence, etc., please print out the applicable forms and note any changes. It is very important that you complete these forms and bring them to your next scheduled visit.
